The Central and North West London NHS Foundation Trust is seeking a Band 5 Community Mental Health Nurse to join the Kensington and Chelsea older adult community mental health team. The role involves working alongside a supportive multi-disciplinary team to provide care for older adults with mental health needs. Responsibilities include assessing, planning, and implementing evidence-based nursing care, conducting comprehensive mental health assessments, and managing a caseload of clients in the community. The post holder will ensure that care plans are tailored to the diverse needs of service users and provide ongoing risk assessments, therapeutic interventions, and collaboration with external agencies.

The position emphasises the importance of cultural competence and the need for a holistic approach to care, including addressing health, social care, and other welfare issues. The successful candidate will also have the opportunity to engage in research and service development activities, and there is no requirement for a driving license as services will be accessed via public transport within this vibrant borough.
