John Smith Medical Centre, in collaboration with Operose Health, is seeking a Practice Nurse to join their team for 21.5 hours per week. The role involves assisting the registered manager in maintaining compliance with CQC outcomes, collaborating with the general practice team, and providing patient care, including chronic disease management and treatment evaluation.

Key duties will include:
- You will assist the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained. As well as working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.
- Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.
