The Patient Flow Team are looking for an enthusiastic and experienced nurse to join the existing team of In-Reach Practitioners that support withassessing patients at the acute hospitals at East Surrey Hospital, ascertaining their suitability for inpatient rehabilitation and supportingwith the Discharge to Assess Model.
The In-Reach Practitioner will support the Site Capacity & Flow Lead in managing patient flow from acute providers to and across SCFT Intermediate Care Units.
The In-Reach Practitioner will ensure all bed capacity across the Trust is utilised whilst ensuring patients experience safe, high quality care throughout their patient journey.
To work with system partners and colleagues to expediate safe transfers and provide safe alternative pathways for patients.
To work collaboratively with external stakeholders and partner organisation's to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised and patient flow is maximised.
To manage daily referrals from all stakeholders
To work collaboratively with the acute providers by providing In-Reach assessments for patients referred to SCFT for a period of in-patient rehabilitation, across the five acute hospitals
To contribute in the management of the waiting list of patients awaiting discharge from the acute providers
To use clinical expertise to be able to support colleagues in the placement of patients awaiting transfer of care
Follow the agreed escalation processes in relation to patient flow and surge escalation plans
To liaise with intermediate care units staff within the allocation of patient to the respective beds, ensuring open dialogue is maintained at all times
To work with acute trusts & all partner organisations to maximise capacity by looking maximising patient flow
Participate in teleconference calls both internally & externally as requested by line manager
To be a signatory for financial payments and other ad hoc purchases within the team, e.g. signing bank timesheets
Support the Patient Flow Administrators (PFA) on a daily basis and carry out PDP's accordingly as requested by line manager
Train and support new staff members ensuring they have SMART objectives set within acceptable timeframes
Full training will be provided, but experience in patient assessment and discharge pathways would be an advantage.
