Job Summary:
We are seeking a part-time Advanced Nurse Practitioner to work 24 hours over 3 days. In this role, you will have the opportunity to assess, diagnose, and treat patients with minor illnesses, ensuring timely access to care and reducing unnecessary GP referrals.

Caseload Management
:
Autonomously assess and manage patients, prescribing evidence-based treatments and making necessary referrals.
Collaboration: Work alongside a multidisciplinary team to ensure comprehensive and holistic patient care.
Patient Consultations: Provide primary care for patients presenting with various conditions through face-to-face, video, and phone consultations.
Caseload Management: Autonomously assess and manage patients, prescribing evidence-based treatments and making necessary referrals.
Care Planning: Develop personalized health plans focused on prevention and self-care, with ongoing follow-up.
Health Promotion: Encourage healthy lifestyles and participate in health promotion initiatives within the practice.
Mental Health Support: Assess and refer patients with mental health needs, collaborating with our mental health team and external partners.
Collaboration: Work alongside a multidisciplinary team to ensure comprehensive and holistic patient care.
Record Keeping: Maintain accurate patient records using SystmOne.
Chronic Disease Management: Engage in chronic disease management and contribute to meeting QOF targets.
Clinical Audits: Participate in clinical audits and assist in developing practice protocols.
