This post holder will be the first point of contact for patients to provide care co-ordination and high-quality patient care. This includes direct patient contact, responding appropriately to sensitive/ complex issues and ensuring confidentiality is maintained.

Care coordination and extensive administrative work supports the MacmillanCancer Nursing Teams to see a larger number of patients efficiently and smoothly, utilising basic clinical knowledge, approved tools and procedures.

The role assists the CNS teams and departments in service developments and supports the teams to implement new initiatives such as developing patient health and wellbeing clinics and other nurse/consultant led clinics. The post holder is instrumental in implementing and embedding electronic CNS team patient records and achieving personalised care and support planning within the patient care pathway

The post holder has effective communication skills in exploring patient situations, this is done solely when triaging the patient telephone helpline where situations are often multi-stranded, complex and delicate.

Reassures patients/carers who may be upset or frustrated, using a range of communication skills; for example when patients are expressing worry and concern about disease and/or waiting time for investigations or treatments.

Acts professionally and appropriately in challenging situations using a range of communication methods/styles enhanced by underpinning knowledge and experience of the patient pathway; such as when patient appointments are changed due to expected results not been available, escalating to a senior nurse when intervention is required.

Using their clinical knowledge and awareness, effectively risk assesses and appropriately responds to any patient situation and rapidly communicates complex, urgent, distressing or emergency situations that may be identified on the telephone or in reception to the relevant nursing member without delay.

As the first point of contact for patients, communicates and coordinates care with several health and social care professionals at all levels including senior clinicians and managers within the Trust.
