The Community Healthcare Assistant position with Norfolk Community Health and Care NHS Trust in Thetford is a newly established role within community nursing teams in the Breckland area. The selected candidate will visit patients in care homes, providing various types of care, such as diabetes management, wound care, venepuncture, and catheter care.

Key responsibilities include planning visits, documenting patient interactions, collaborating with healthcare professionals, and applying clinical techniques under supervision. The role requires a basic understanding of health care, flexibility, and the ability to communicate effectively with patients facing different health challenges.

Additionally, the assistant will contribute to patient education, maintain equipment, support safe practices, and assist in discharge and admission prevention efforts. The role involves both independent work and part of a team, necessitating participation in training, appraisals, and adherence to organisational policies.
