The Birmingham Women's and Children's NHS Foundation Trust is seeking a Regional Transition Pathway Development Nurse to join its specialist nursing team within the Midlands regional haemoglobinopathy service. This dynamic role will be split between Birmingham Children's Hospital and City Hospital Birmingham, requiring the post holder to collaborate with various stakeholders to enhance healthcare transition pathways for children and young people with red cell disorders. Responsibilities include managing regional service reviews, developing and implementing a robust transition policy, advocating for patients and families, and providing professional guidance during Multi-Disciplinary Team meetings. The position also involves training and development efforts with regional education lead nurses to ensure effective transition practices.
