The A31 Alton Primary Care Network has an exciting opportunity for you to join our team
We are looking for a Registered Nurse to join our dedicated and highly professional team of Nurses working across the Alton and surrounding areas. The Alton Primary Care Network serves three of the registered General Practices and you will play a pivotal role in keeping hospital admissions and re-admissions to a minimum as well as encompassing the dimensions of health service planning, advocacy and care planning for patients with a wide range of service needs.
Within this role, you will act as part of an ever-expanding multidisciplinary team, developing collaborative, needs based care plans, through assessment, planning, implementation and evaluation.
You will be required to provide important educational and advisory service for patients, offering information on various aspects of health care to help support patients living with their condition. Within this role, you will be expected to provide nursing intervention to patients which include; wound management, catheter care, medicines management as well as palliative care.
You will need to be effective in communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service.
We also offer our staff bespoke training packages, supportive team working environment, leadership pathways and comprehensive preceptorship programme for those that are newly qualified.
