The position supports a range of shift patterns, with opportunities available for part-time work. The REACT team emphasises staff well-being, continuous professional development, and effective communication skills in managing non-complex patient cases and ensuring adherence to trust policies for safe discharges.
REACT are looking for a new Assistant Practitioner to join our team!
Our team consists of Occupational Therapists, Physiotherapists and Assistant Practitioners, and our interdisciplinary way of working aims to avoid hospital admissions for those patients attending 'The Front' Door whilst providing high standards of therapy, care and discharge planning.
We work within A&E, the Medical Assessment Unit and Acute Medical Short Stay Units, including Acute Frailty. This means that successful candidates will have the opportunity to work as part of the REACT team in order to manage their own caseload within a fast-paced but extremely rewarding environment.
Due to ongoing service developments there are lots of opportunities to develop skills that will enable you to progress your career within an Emergency Medicine setting.
We strive to celebrate our team success whilst ensuring staff and team wellbeing is kept at the forefront of what we do. We offer regular supervisions and a team in-service training programme in order to support this alongside professional development. Applicants should have a Foundation degree/equivalent level 5 qualification or be willing to work towards this within the first 12 months of employment.
Our service covers 7 days and extended hours, enabling a variety of shift and working patterns. Part-time applications will also be considered.

Successful candidates will work alongside an experienced, knowledgeable and supportive team in order to provide comprehensive assessments, therapy intervention and discharge planning for patients attending 'The Front Door'.
REACT are passionate about reducing the risk of deconditioning whilst in hospital and we therefore support with therapy input alongside our discharge planning processes.
Whilst doing this, successful candidates will build good working relationships with the wider multidisciplinary teams and external partners in order to devise effective discharge planning whilst improving patient experience. This therefore means that applicants should have excellent communication and team working skills.

About us
- To work autonomously within the Emergency/Acute Medicine Departments demonstrating effective time management skills to manage a varied caseload of non-complex patients.
- To screen and assess patients needs, devise and implement treatment programmes and identify appropriate discharge pathways with reference to a senior member of the team.
- To contribute to effective discharges, promoting admission avoidance/home first approach as appropriate, always ensuring adherence to Trust policy.
- To routinely act as the first contact for patients referred to REACT identifying the complexity of each case.
- To assist senior members of the team in the delivery of the service to a specified caseload of patients.
