The role of "Co-occurring Mental Health & Substance Misuse Practitioner" with Hywel Dda University Health Board in Aberystwyth is focused on supporting individuals who have both mental health issues and substance misuse problems. The practitioner will work within a multi-disciplinary team to assess and manage clients, providing evidence-based interventions such as harm minimisation, relapse prevention, and detoxification programmes. Key responsibilities include maintaining a caseload, conducting assessments and care planning, facilitating access to community resources, and promoting recovery while ensuring effective communication and collaboration with other services.

Additionally, the practitioner will be involved in improving patient outcomes across care pathways, supporting families, and staying informed on relevant policies and legislation. While the ability to communicate in Welsh is desirable, the Health Board encourages all applicants and offers support for those looking to improve their language skills.
