Mental Health Practitioner - LD Nurse 37.5hrs per week Permanent The post holder will work as part of a multidisciplinary CAMHS team in Liverpool. They will work to engage children, young people with a learning disability and their families in the therapeutic process, supporting the assessment and implementation, under the supervision of an experienced CAMHS clinician, of complex mental health interventions for children and young people The post holder will be expected to liaise with external agencies including health, education, social care and the voluntary sector and act as a named key worker to children, young people and families accessing Alder Hey CAMH services. Under supervision the post holder will carry out effective and evaluated assessments in line with their own knowledge base and the trust's policies and procedures. The post holder will also be responsible for enabling effective and meaningful service user participation in the design and delivery of our services through individual and group engagement, facilitation of events/focus groups, supporting the wider CAMHS participation agenda and that of Alder Hey Children's NHS Foundation Trust. The post holder will have the ability to self-reflect and be aware to work within their job remit. They will also attend team meetings, regular supervision and will be willing to joint work cases with all members of the multidisciplinary team Main duties of the job To work directly with children and young people with a learning disability and their families as directed by senior clinicians. To maintain patient records (including care plans and risk assessments) in line with trust policy. To escalate any risk or safeguarding concerns to senior colleagues and other relevant agencies in line with trust policy. To work in the community, including home visits and through agreed activities to positively improve a young person's social and emotional development. To work flexibly in order to maximise opportunities for engagement of children, young people and families. To develop positive relationships with children, young people and families which actively supports their engagement. To identify appropriate activities for young people and develop a local knowledge of youth groups and alternative voluntary projects young people can link with.
