The role of Community Learning Disabilities Nurse at Mersey Care NHS Foundation Trust in Liverpool involves working within a dynamic community team to support adults with learning disabilities who also have complex physical and mental health conditions. The nurse will manage a caseload with supervision and participate in an on-call rota, providing care through various pathways and facilitating access to services.
