CASELOAD HOLISTIC MULTI-DISCIPLINARY NHS ROLE MODEL
The Team Leader for the Hospital Palliative Care Team at Queen Elizabeth Hospital, Woolwich, will spearhead the delivery of high-quality, patient-centered specialist palliative care services. This role emphasises collaboration with Lewisham and Greenwich NHS Trust to support patients with life-limiting conditions, their families, and caregivers. Key responsibilities include mentoring team members, managing operational aspects such as budget oversight and service provision, and fostering interdisciplinary collaboration across hospitals, hospices, and community care settings. The leader will also focus on driving quality improvements, advocating for best practices in palliative care, and participating in or leading educational and research initiatives.

Additionally, the Team Leader will ensure continuous learning and professional development within the team while actively supporting patients and families through compassionate advocacy. They will oversee the implementation of high standards of care, facilitate clear communication among multidisciplinary teams, and promote awareness of bereavement support services. Overall, this role combines clinical practice, management, and advocacy to enhance palliative care services within the hospital setting.
