Following a retendering process, the children's service is due to be transferred to the new provider on the 1st of April 2025. Given this post sits within the transferring service, in line with the legislative requirements, the successful candidate will be part of the TUPE transfer along with other colleagues.

This is an exciting challenge for a motivated NMC registered Nurse or Midwife to join our experienced skill mixed 0-19 team based in Dorking, Leatherhead, Banstead and Tattenham.

The ideal candidate for our team would be a clinical practitioner with excellent interpersonal and communication skills. We look for individuals with a strong approach to teamwork, those who demonstrate initiative, a good understanding of children, young people and their families and ability to support families from a diverse population within the borough.

You will work alongside Health Visitors and will also be supported by our Clinical Service Managers, Clinical Team Leaders and our Safeguarding Team to provide excellent professional practice and to further develop your skills and knowledge.

About us

Purpose

The post holder will be a member of the 0-19 public health team delivering the 0-19 Healthy Child Programme. Supported by the Specialist Community Public Health Nurse(SCPHN), the post holder will participate in delivering a universal core service, provide targeted interventions, to address health inequalities and where necessary referring to specialist services. This role will also include safeguarding responsibilities, mentorship of students and staff appraisals.

Principal duties to include:

Undertake duties as allocated by the SCPHN.

Build and develop good working relationships the wider multi-disciplinary team.

Make contact with children and families in a variety of community settings e.g. home, children centres, child health drop-ins

To undertake development assessments when trained to do so

To deliver Health Promotion activities to parents and young children

To carry out visits to review aspects of development as directed by the SCPHN

To manage a delegated case load

Assist in the delivery of the Healthy Child Programme 0-19

Provide evidence based care to parents and children e.g. breast feeding , behaviour management

Provide enuresis advice and continence assessments

Identify and support children and families through health needs assessments who have unmet health needs, provide support and targeted interventions and where necessary refer to specialist services

Take appropriate action in cases of suspected or actual child abuse, working within Surrey safeguarding childrens code of practice and procedures.

Attend child protection conferences/core groups as required and participate in safeguarding supervision

Undertake health needs assessments for children who are Looked After

Assist in research and service audits

Supervise junior member of the 0-19 team as directed by the SCPHN. Undertake junior staff PDR appraisals and direct line management.

Take an active part in clinical supervision

Participate in meetings as appropriate

Be prepared to be flexible within contracted hours

In accordance with the needs of the service, undertake other duties as required

Communication skills

Be skilled at adapting language and professional approach to a varied client group

Be proficient in presenting health promotion sessions to differing sizes of groups.

Be competent in writing concise accurate contemporaneous client records using approved Children and Family Health Surrey templates

Be competent at making referrals to the wider multi-disciplinary team

Demonstrate ability to produce child centred reports for children and families who are subject to safeguarding plans or Child in Need/Looked After Child reviews

Contribute ideas to discussions at health visiting forums and team meetings

Analytical and judgemental skills

Able to understand complex family situations and respond and refer appropriately

Be able to undertake family health needs assessments, using child centred approach to gather health information that identifies any unmet health needs

Contribute to population profiles within local communities

Participate in team/service audits

Planning and organisational skills (f4)

Be responsible for planning own work diary to meet caseload allocated by SCPHN or Clinical Team Leader

Support 0-19 team initiatives to deliver health promotion activities

Prioritise diary to meet needs of families on level 3-4 caseload and assure attendance at safeguarding meetings and safeguarding supervision

Maintain compliance with statutory and mandatory training
