The Care Coordinator position with Pennine Care NHS FT's Early Intervention Team in Ashton-Under-Lyne involves providing essential support to individuals experiencing their first episode of psychosis. This Band 6 role requires a qualified professional (RMN, Social Worker, Occupational Therapist) to manage a caseload of service users in the early stages of psychotic illnesses or at risk mental states. Responsibilities include assessing needs, delivering evidence-based interventions according to NICE guidelines, organizing daily team operations, and advising other multi-disciplinary team members to ensure effective service delivery.

The role also entails contributing to the ongoing development of the service, promoting it to partner agencies, and implementing a psychosocial and recovery-oriented approach in all interventions. The Care Coordinator will work collaboratively to enhance the overall support provided to service users, ensuring their needs are met within a structured clinical environment.

The Early Intervention Team provides support and intervention to individuals who are experiencing a First Episode of Psychosis. The team is based in an office with purpose built clinical space in the centre of Ashton-Under-Lyne, covering the Tameside and Glossop area and has access to remote working systems. This is an exciting opportunity secondment opportunity for the post of Band 6 Care coordinator (RMN, Social Worker, Occupational Therapist) to work within this established specialist service.

As a Care Coordinator you will hold a caseload for service users identified as being in the early phase of a psychotic illness or an At Risk Mental State. This will include the assessment of needs and delivery of interventions in line with currently identified best practice and in line with NICE guidance. Supported by the Team Manager you will support the day to day organisation of the team and ensure that work priorities are met, providing advice and supervision as required to other mental health professionals within the multi-disciplinary team and contributing to ongoing service developments
