Vine Medical Group is seeking a Care Navigator (Medical Receptionist) to join their Care Navigation Team in Waterlooville. This role involves acting as the first point of contact for patients, both in-person and over the phone, providing high-quality support to help them navigate the healthcare system and access the necessary services efficiently and courteously. Candidates should possess strong interpersonal skills, a patient-centered approach, and the ability to communicate effectively with diverse individuals in a fast-paced environment. The position requires working 30.5 hours per week, with a small commitment to occasional Saturday shifts.

Responsibilities include greeting patients warmly, managing queries, maintaining knowledge of practice services, assisting with digital appointment requests, and ensuring confidentiality. Additionally, the Care Navigator will help facilitate communications between patients and healthcare professionals and support the overall operations of the practice. The role is crucial in enhancing patient access and continuity of care within the Primary Care Network.
