                                                    Practice Nurse - Rock Healthcare with BARDOC in Bury

                                                     Are you a Practice Nurse interested in working in a long established, forward thinking and dynamic Family Practice? We would love to hear from you! You will be required to work 30 hours per week [ Monday - Friday].   The post holder is responsible for the delivery of day-to-day practice nursing services to the practice population. Supported by the clinical team within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities both in Practice and in the community. They will work collaboratively with the general practice team to meet the needs of the patients and to refer into complementary services as required; supporting the delivery of policy and procedures and providing nurse leadership as required.   About us  The focus of the role is both the delivery of evidence-based practice for Patients presenting with a long term condition, and the provision of preventative health care to the Practice population. As an autonomous Practitioner, the Practice Nurse is responsible for the care delivered to the population of Bury, demonstrating critical thinking and skills in clinical decision-making in the management of Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery of a healthier Bury through listening and responding to Patients needs.
