This role is for a period of 6 months to cover the current post holder career break. To provide a high quality service across Lewisham and Greenwich Healthcare NHS Trust. Through line management of the Neighbourhood Nurse Leads and working in close collaboration with other clinical colleagues lead the development in the delivery and expansion of an expert Community Nursing service across the organisation. About us To provide autonomous, specialist, self-directed care and treatment to patients within a defined area or caseload. Admitting and discharging patients from own caseload as appropriate. To provide and undertake home community visits that will deliver direct clinical care, monitoring efficacy and compliance of patients undergoing treatment. To undertake advanced clinical care, interventions and procedures commensurate and as required within the specialty. To provide expert evidence based advice to patients, carers and other healthcare professionals. Employ effective decision making skills to address complex, distressing and sensitive issues and use effective change management skills to complement them. Undertake comprehensive assessment of patient health needs. Assess, monitor and evaluate disease activity and drug therapy of patients where necessary by initiating and interpreting clinical data, explaining treatment goals and managing side effects. To act as the point of contact for the service. To act as an advocate to empower patients, their families and other carers to participate in decisions regarding their care. Engage patients and carers/families in the planning and delivery of care and the development of services to enhance the patient experience. To ensure optimum liaison and co-ordination of care for patients between primary, community and secondary care. To participate in the business planning process for the service participating in the formation of business cases. Lead the nursing service review and redesign of pathways ensuring consistency across the organisation and parity of access. To develop, influence and update policy, guidelines/protocols and integrated care pathways within the service and implement service improvement, while ensuring a high standard of care continues through ongoing review. To provide expert clinical information and advice to patients, carers and GPs and staff through telephone advice lines / email, which is appropriate and responsive. To provide effective teaching to individual patients and their families about their condition, treatment options and possible side effects. Act as lead investigator to selected clinical incidents and provide reports to a high standard. Undertake root cause and trend analyses for complaints and incidents relating to relate to nursing or midwifery, and participate in SI investigations. To manage and maintain any disease specific registers as required by the service and participate in National and Local audits and peer reviews as required. Challenge poor practice. Prioritise own workload and manage time effectively, ensuring integration with existing support networks. Identify risks within the area of responsibility and identify, plan, implement and evaluate strategies to reduce and manage risk. To influence strategic development to advance quality of care and enhance the patient experience. Utilise research findings, as applicable, in the delivery of specialist patient care, disseminating relevant information to colleagues Contribute to the development of research / evidence based practice within the team. Identify areas for and participate in nursing and multi-disciplinary research Identify areas for and actively participate in clinical audit. Take responsibility for maintaining clinical expertise in relation to current research and evidence based practice. To represent the Trust at Network meetings. Work force Act as a resource to others by providing specialist teaching and improving knowledge and skills. Act as a role model, supervise, mentor and offer peer support to colleagues, junior staff and students in line with Trust Values and Behaviours. To share and develop good practice and promote evidence based practice. Develop strategies to communicate new developments to staff and other colleagues actively. To line manage the team and develop the nursing/midwifery service within the specialty ensuring that performance, appraisals and mandatory training is in accordance with Trust standards. To participate in the development and delivery of pre-registration and post-registration education effectively in order to deliver the Trusts objectives
