The AACC Case Practitioner position at Hampshire and Isle of Wight ICB involves supporting individuals of all ages who require continuing care, ensuring they receive optimal support. This dynamic role offers hybrid working flexibility, allowing employees to balance office and home work environments. Responsibilities include providing non-clinical support to senior nurse assessors in the assessment, review, and case management processes, preparing funding requests, liaising with patients and care agencies, and updating care management systems. The role emphasises continuous learning, teamwork, and the importance of effective communication.
