An exciting opportunity has arisen for Band 3 Community Health Care Support Worker based in the Tenby District Nursing Team. This is a part-time position working 33 hours per week.

As a senior healthcare support worker you will be responsible for providing high quality care to patients in their own homes as part of a nursing team, with the guidance and support of an experienced District Nurse Team Leader or designated representative.

The post holder will ensure high standards of individualised care are delivered to patients and their families and carers within the home or clinic environment, by following care plans and treatment interventions as delegated by the registered nurse, as well as having the ability to work competently without direct supervision.

Pembrokeshire's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.

This is a permanent position worked over a 7-day period including weekends and bank holidays.

The post holder will need the ability to travel between sites in a timely manner.
