Role Summary:
To be a team member within Alder Hey's Development Paediatrics Team providing a substantial contribution to the clinical work of the team: including accepting, prioritising and assessing referrals of children and their families. Providing assessment and planning arrangements.
This role will require the post holder to provide clinical management and co- ordination of complex referral pathways, whilst working collaboratively within a wider multi-organisational team. You will work to develop a robust referral process as part of a multi-disciplinary team.
You will work in partnership with other health professionals, parents, carers, education and other service providers to ensure a child centred approach to receiving healthcare.
In addition, this will include liaising with referrers for additional information, or signposting to the most appropriate service.
To function as an autonomous senior member of the MDT using experience
and knowledge of specialist pathways, out of hospital provision as well as needing to influence referral decisions.
Liaise with referrers and support services to effectively signpost child/young person to the correct service.
Being able to recognise and respond effectively to safeguarding issues, adhering to Alder Hey policy and procedure.
Utilising analytical skills, specialist knowledge and experience provide a comprehensive assessment and formulation of the presenting problem taking into account the child's educational attainment, chronological/ developmental stage, inner world/ emotional wellbeing, the mental health of the child's parents, the family dynamics and environment, and the broader social context including school.
To adopt a child/young person centred and sensitive approach to care, collaborating with children, parents and / or carers and education staff in all aspects of the referral process.
To participate in team meetings and multi-disciplinary meetings as required occasionally being required to assertively represent the outcomes of referrals and where appropriate challenge the views of other professionals based on the evidence and reasoned views arising from knowledge and referral criteria.
To contribute to and receive information as part of a multi-disciplinary/multi- agency team to provide a co-ordinated and effective service to child/young person and carers.
To communicate accurate and sensitive information to referrers and carers using an appropriate means of communication.
Deal effectively and efficiently with any enquiries (telephone or face to face) from clinical and non-clinical staff within the Trust in a professional manner.
Deal with routine telephone enquiries from external agencies in a professional, polite, and confidential manner, referring to appropriate person/department regarding any enquiries outside of the post holders remit.
Attend referral meetings and contribute to establishing the information required to ensure the child/young person is in the correct pathway.
Establish and maintain positive & effective working relationships within the wider multi-organisational team environment both with immediate colleagues and members of remotely based teams.
Ability to manage distress and conflict when working with children/young people/parents/guardians who, as an expression of their distress following a declined referral, may at times be difficult to engage and agree health pathway for the patient.
To ensure that up to date records and activity data are maintained in accordance with professional and Trust standards and policies.
To attend and chair referral meetings and facilitate multidisciplinary pathways to support a safe and smooth referral process ensuring the holistic needs of the child or young person are addressed.
