Devon County Council is seeking a dedicated Mental Health Adult Transitions Social Worker to join the Transition in Adult Social Care (TASC) Team in Exeter. The role involves working closely with young people facing mental health challenges, their families, and a multidisciplinary team to facilitate seamless transitions from children's to adult services. The social worker will be essential in collaborating with Children and Adolescent Mental Health Services (CAMHS) and other agencies to promote independence and holistic well-being for young individuals aged 14 to 25. The position offers practice support from Devon Partnership Trust (DPT) Social Care and emphasises the importance of integrated services to improve outcomes for young people making this crucial transition into adulthood.
