To ensure repeat medication is issued in line with the protocols that are already in place, and where queries are raised that they are dealt with in the best way.

We are seeking an experienced prescription clerk, to join our friendly prescribing team. The successful candidate will play a crucial role in ensuring the smooth and efficient operation of our prescription system.

  • Support the Pharmacy team and GPs in managing repeat prescriptions and medication queries for the 2 practices within West & Central PCN.
  • Preserve complete confidentiality of patients records and clinically related matters at all times.
  • Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies.
  • Undertake a variety of administrative duties to assist in the smooth running of the practice including the provision of clerical support to clinical staff and other members of the practice team.
  • Data entry onto patient records including significant use of EPS.
  • Comply with legal and professional requirements, accepted guidance on professional practice, and with safe systems of work for re-authorising repeat prescriptions.

Prescriptions:

  • To process, all medication requests sent in by patients using the various protocols in place.

  • Monitor the prescription email, online patient access for patients medication request and queries.

  • Ensuring that all paper requests are process daily.

  • Process patients medication on set days (on wall calendar)

  • Print patients Dossett box on set days for the pharmacies.

  • Working alongside with the practice pharmacist with queries to solve the issue together.
  • Following up prescription requests/queries via the practice task system
  • Liaise with the pharmacy when there are Dossett box changes via telephone and email.

  • Answering the telephone to patients and other healthcare professionals and helping with any queries efficiently.

  • To liaise with patients and chemists regarding queries and requests.
  • To ensure repeat medication is issued in line with the protocols that are already in place, and where queries are raised that they are dealt with in the best way. Following protocols for monitored medication (i.e. DMARDs, ADHD, Warfarin).
  • To ensure that patient queries on medication are highlighted to the relevant GP or practice pharmacist.
  • On a regular basis goes through prescriptions not picked up and unlikely to be picked up as a result of the timescale, to remove the details from the patient records and then destroy the prescription.
  • Inputting patient data (i.e. Warfarin details, blood results)
  • Liaising with hospitals re medication summaries/queries.
  • Sending out appropriate letters (i.e. new repeat slips; repeat dispensing; medication reviews)

  • Expedite urgent prescriptions to help ensure a safe and responsive service for our Patients.
  • Ensure you are keeping Patient information updated and that Patient information is secure /kept confidential at all times.
  • Maintain accurate Patient records promptly and quickly - entering data in accordance with reporting requirements.
  • Provide an efficient administration service as part of a Team to ensure the smooth running of the prescription support to our clinicians
  • Respond to enquiries from Patients, carers, secretaries, NHS administrators, Consultants or the Medicines Management Team in a courteous, timely and efficient manner, by telephone, letter and email, occasionally face-to-face and maintain precise records of the communications
  • Issuing appliance items for patient upon request from appliance companies (catheters, dressings)
  • Build good working relationships with local Pharmacies to help enable our prescription service
  • Chasing Hospitals and other healthcare organisations for patient information such as up to date letters, Discharge, querying information given by them via email and telephone.
  • Medicine management audits as requested by GP Practice Manager - ICB Prescribing advisor


Searches

  • Ensuring that this is done monthly
  • Sending out the correct blood test and information (related to the search)
  • Making sure patients health prompts (weight, height, BP) has been checked recently to allow pharmacist to complete the medication review.
  • Ensuring that all searches are completed monthly for the GP and practice pharmacist to then complete their part.


Results

  • Answering the results line and informing patient of result.
  • Chasing the result for the patient
  • Results received via the link
  • Ensure patients are matched
  • Ensuring doctors are matched
  • Ensuring results for GPs on leave are forwarded to another GP
  • Rejecting results for patients that are not registered ensuring the hospital is notified via emailed
  • Checking the cytology results and informing the GP of abnormal results and adding the relevant diary date for recall.
