Hastings & Rother Healthcare is seeking a full or part-time Extended Scope Nurse or Paramedic specializing in Frailty and Palliative Care. The ideal candidate should be passionate about delivering high-quality, patient-centered care within a collaborative multi-disciplinary team environment. The role involves conducting home visits, face-to-face consultations, and telephone assessments for patients with varied health concerns, managing urgent requests and chronic disease care, while ensuring safe clinical decision-making and effective treatment evaluation.

Key responsibilities include performing holistic assessments, supporting and coordinating care for Palliative and End of Life patients, and caring for frail and elderly individuals, including residents in care and nursing homes. The position requires expertise in physical assessment and diagnostic skills, prioritizing patient needs, and making appropriate referrals.
