Sheffield Churches Council for Community Care is seeking a compassionate and self-motivated Home from Hospital Discharge Support and Admission Prevention Worker to join their team in Doncaster. The role involves providing vital support to older and vulnerable individuals post-discharge from hospital, ensuring a safe transition home and preventing hospital readmission. Responsibilities include pre-discharge planning, arranging necessary support and utility checks, offering post-discharge assistance, and facilitating connections to community resources.

The position requires flexibility in working hours, with shifts released on a 4-week basis, and operates seven days a week. Candidates will work collaboratively with health and social care professionals as well as families to deliver individualized, low-level practical support. This role is crucial for helping individuals regain confidence in living independently while reintegrating into their community. Maintaining accurate records, providing ongoing support to volunteers, and adhering to organisational policies are also key components of the role.
