Tees Esk and Wear Valleys NHS Foundation Trust seeks a Senior Community Mental Health Practitioner for its Community Mental Health Team in York, aimed at supporting older adults with mental health needs. The role involves working within a multi-disciplinary team to provide assessments, care coordination, and interventions for individuals with diverse complexities, ranging from mild to severe mental health issues. Candidates should have at least 18 months post-qualifying experience, alongside strong assessment, interpersonal, and collaborative skills, with an emphasis on working closely with older adults and their families.
