CASELOAD FAMILY MENTAL HEALTH MULTI-DISCIPLINARY NHS
The Lincolnshire Partnership NHS Foundation Trust is seeking a full-time Band 6 Community Mental Health Practitioner in Stamford. This role involves transforming adult community mental health services by providing recovery-focused support and interventions that prioritize patients' individual needs and promote their independence. Responsibilities include conducting assessments, delivering therapeutic interventions, managing care plans, and ensuring effective communication between hospital and community services. The practitioner will also collaborate with multidisciplinary teams, manage complex caseloads, oversee the safety and quality of care, and support both clients and their families.

In addition to patient care, the role includes responsibilities such as risk assessment and management, medication administration, and providing managerial supervision to team members. The practitioner will work flexibly across a 7-day service, adhere to professional and ethical standards, and engage in clinical governance activities. Through effective relationship-building within the local healthcare system, the aim is to enhance the wellbeing and quality of life for individuals accessing mental health services in the Stamford and Bourne areas.
