The University Hospitals Dorset NHS Foundation Trust is seeking a part-time Band 6 Senior Occupational Therapist to join their dynamic Interim Care Team in Christchurch. This role involves working with a multidisciplinary team to facilitate a discharge-to-assess service for patients transitioning from hospital to their homes, emphasizing the safe and effective flow of patients. Candidates should possess community and leadership skills, as well as HCPC registration, to provide expert clinical support and manage a caseload of elderly patients, all while implementing Occupational Therapy treatment programs aimed at enhancing patient independence and quality of life.

The position includes some weekend work on a shared rota and focuses on performing autonomous assessments of patients' functional levels, setting achievable goals, and coordinating with community therapy assistants to deliver high-quality care. The Interim Care Team is devoted to improving health and social care integration in Dorset and supports patients with both physical and mental health conditions in their recovery journey at home.
