The Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Community Mental Health Practitioner for their Chester Community Mental Health Team. The role involves working within a multi-disciplinary framework to assess, coordinate, and deliver care to patients with various mental health disorders.
