We are looking for a Band 4 Community Nurse Associate to work within the Out of Hours Countywide community nursing team supporting patients with clinicalhealth needs. As a nursing associate, you will be working within the multi-disciplinary team and neighbourhood team. You will provide high standard of nursingcare and interventions within the patient's own home, with support from the wider team. The patient will be at the centre of the care beingprovided, ensuring patient and carer participation in decision making. We provide a service seven days a week, working closely with other community services and PCN's to provide high standard care. You will have the opportunity to develop yourself, with extensive learning and development opportunities. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To support, educate and advise formal and informal carers with the nursing care which they can provide. To attend all relevant mandatory training as and when required to do so.
