The role of Primary Care Pathway Manager in Bradford with NHS West Yorkshire Integrated Care Board focuses on supporting the Planned Care (PLDC) and Data Quality (DQ) initiatives within the Bradford District and Craven area. This position involves collaborating with various stakeholders, including the GPIT Clinical Lead and DQ Teams, to develop, maintain, and optimize the Assist initiative, which encompasses Care Pathways and Referral Management. The manager will ensure compliance with Care Pathway Protocols, promote best practices, and facilitate the governance of the Assist system. Key responsibilities include communicating complex information effectively, managing project tasks, providing training, and driving digital innovation and research in healthcare.

The successful candidate should have extensive experience with SystmOne and a strong attention to detail, as well as advanced communication and interpersonal skills. They will maintain relationships with a variety of stakeholders, including ICS Senior Managers, General Practices, and NHS Digital. Proficiency in data manipulation and the ability to propose policy or procedural changes are also essential. The role requires travel across the ICB, necessitating access to personal transportation.
