The Mid Yorkshire Teaching NHS Trust is seeking Discharge Coordinators to enhance patient flow and experience during transitions of care at their acute hospital. The role involves coordinating discharges in collaboration with a multi-disciplinary team, ensuring safe and timely transfer of patients, and actively participating in ward-based discharge activities. The ideal candidates will have knowledge of the health and social care environment, strong communication skills, and the ability to manage complex cases effectively. This position operates seven days a week across all hospital sites, emphasizing teamwork and proactive discharge planning.

Key responsibilities include promoting best practices in discharge processes, participating in ward activities to identify patients for transfer, and maintaining effective communication with internal and external partners to reduce delays. The role requires problem-solving skills to develop management plans and support staff in challenging discharge situations.
