The Community Mental Health Nurse position within the Recovery and Wellbeing Team at Essex Partnership University NHS Foundation Trust in Brentwood offers a dynamic and rewarding opportunity for an enthusiastic individual passionate about community mental health. The role involves providing high-quality community care primarily in clients' homes, focusing on the assessment, coordination, development, implementation, and evaluation of care programs for adults experiencing mental health issues. The nurse will work closely with a multi-disciplinary team, including GPs and other healthcare professionals, to ensure an integrated service while maintaining care standards within the community mental health framework.

Key responsibilities include managing patient caseloads through holistic assessments, planning interventions, and accurately documenting care. The position also requires participation in initial client assessments for suitability for services, engaging in team and clinical meetings to discuss care improvements, and maintaining effective risk management by monitoring clients' mental states. The role reports to the Clinical Lead on a day-to-day basis and is ultimately accountable to the Team Manager, fostering an environment of collaboration and excellence in mental health care.
