We are seeking a Full Time Clinical Pharmacist to work five days a week across six practices within the Moseley, Billesley & Yardley Wood Primary Care Network (MOBY PCN).
The practices are based within B13, B14, B27 & B28. Please note this role is to be based within practices and not worked remotely.
We are looking for a Clinical Pharmacist who has ideally completed the CPPE Pathway, holds an IP qualification and is actively prescribing.
This is an exciting opportunity to join our existing team of Clinical Pharmacists. Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacist in the PCN.

The position of Primary Care Network clinical pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for a clinical pharmacist to work with existing practice and PCN teams to deliver consistency and excellence in medicines optimisation across the member practices.
We are looking for a pharmacist who will work as part of a multi-disciplinary team, developing and managing medicines optimisation services within our practices. Demonstrating improvements in value and patient outcomes and contributing to reducing health inequalities through the safe and appropriate use of medicines. This includes implementation of systematic processes for improving medicines optimisation across our PCNs and the wider OHP network. Work programmes will be prioritised according to the population needs of the PCN.
The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.
