Our Health Partnership is seeking a full-time Clinical Pharmacist to join the Moseley, Billesley & Yardley Wood Primary Care Network (MOBY PCN) in Birmingham. This role requires the pharmacist to work across six practices within the designated areas, focusing on improving patient care through effective medicines optimisation. Candidates should ideally have completed the CPPE Pathway, possess an Independent Prescribing qualification, and be actively prescribing. The pharmacist will collaborate with multi-disciplinary teams, supporting general practice staff with prescription queries, conducting medicines reconciliation, and implementing systems to enhance safe prescribing practices.

The position is integral to delivering high-quality healthcare and addressing health inequalities in the community through the optimal use of medications. The successful applicant will work closely with the Consultant Pharmacist and the Lead Clinical Pharmacist, contributing to the development and management of medicines optimisation services tailored to the specific needs of the PCN population. This is an in-practice role, emphasizing teamwork and professional collaboration to achieve improved patient outcomes and consistent healthcare delivery.
