The East Cheshire NHS Trust is seeking two experienced Cardiac Rehabilitation Nurses for the role of Associate Specialist Practitioner – Cardiac Rehab, available exclusively to current staff within the Trust, including bank staff and volunteers, for 22.5 and 14 hours per week. Candidates should possess significant post-registration experience in cardiac rehabilitation and cardiology, with a commitment to ongoing professional development. The role involves providing effective cardiac rehabilitation to patients from referral to discharge, autonomously managing a caseload while contributing to a multidisciplinary team, delivering nurse-led clinics, and conducting risk assessments to ensure patient safety.

Ideal applicants will have excellent communication, organisational, and time management skills, alongside a keen interest in behavioral change strategies for cardiac patients. Responsibilities include running individual and group rehabilitation programs, participating in clinical audits, and contributing to both patient education and the development of healthcare professionals.
