An exciting opportunity has arisen for 2 experienced Cardiac Rehabilitation Nurses to join our friendly, driven, passionate and nationally accredited Cardiac Rehabilitation service. The posts available are 22.5 hours per week and 14 hours per week.
You should have significant relevant post registration experience including within cardiac rehabilitation, cardiology with evidence of continued professional development. You should have experienced and have a keen interest and understanding in the rehabilitation of cardiac patients. You should also understand the process of behavioural change for patients to reduce their risk of further cardiac events and assist them in recovering to their best physical, psychological, social and vocational status. You will have excellent communication skills, be organised, a good team player and have time management skills to work under pressure alongside other members of the team. You will be expected to work autonomously and be able to make decisions.
Hours per week - 22.5 & 14
Provide timely and effective cardiac rehab to patients according to clinical protocols and service priorities from initial referral until discharge from the service.
Works autonomously and flexibly to manage own caseload of patients whilst working as part of a multidisciplinary team.
Provide a ward presence and identify appropriate patients for cardiac rehabilitation according to evidence-based guidelines.
Provide nurse-led clinics, performing functional assessment and using specialist knowledge to ensure the correct cardiac rehabilitation pathway for patients.
To manage clinical risk within own caseload of patients and to complete risk assessments to manage risk within their own working day.
To deliver individual and group based cardiac rehabilitation programmes and to be responsible for the safety of patients.
Participate in clinical audit and contributes to development of knowledge within the specialist field of practice.
Contributes to the delivery and development of specialist educational programme for patients and carers.
Supports in the teaching and development of healthcare professions, patients and carers.
