The role of Advanced Clinical Practitioner in the Inpatient Rehabilitation Unit at Sirona care & health CIC in Yate is designed for clinicians with a background in urgent patient care and long-term condition management. The practitioner will primarily work within a multidisciplinary team to provide clinical care for up to 30 patients who are medically stable but require rehabilitation following acute hospital admissions. Responsibilities include assessing new patients, managing their ongoing health needs, optimizing their rehabilitation, and providing urgent medical interventions when necessary.

The position offers a supportive environment to enhance clinical skills and further professional development. The candidate will collaborate with acute hospitals and other system partners while ensuring safe and effective care for patients. It's important to note that this role is classified at AFC Band 7 and does not require a complete MSc in Advanced Practice, although support for further training may be available based on individual learning needs.
