Nottingham University Hospitals NHS Trust is seeking a qualified social worker to join its Adult Cystic Fibrosis team at the Wolfson Centre. This full-time position involves providing psychosocial support to patients with cystic fibrosis, their families, and caregivers across the Nottinghamshire, Derbyshire, and Lincolnshire regions. The role includes conducting home visits, collaborating closely with social services and other professionals, and addressing the social and psychological needs of those affected by this condition. Flexibility in working hours may be available, and the applicant will receive support and training to develop their skills within an established multidisciplinary team.

The social worker will be responsible for managing a caseload, advocating for patients, and engaging in care planning and assessments. Excellent communication skills and a full UK driving license are essential for this role, as well as a willingness to learn and actively participate in service development within a supportive healthcare environment.
