St Pauls Primary Care Network presents an exciting opportunity for an experienced Health Care Assistant to work across the four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham.

The successful candidate will work across the four practices continuing to support the successful home visiting service, and in addition supporting the PCN and Practices with patient annual health checks. This role will be closely supported by the wider PCN team of Allied Health Professionals.

We are offering a part time position of 24 hours a week.

  • Provide direct care personalised support to patients needing Long Term Condition and Health Checks particularly specific hard to reach groups such as: Learning Disability, Serious Mental Illness, Homebound Patients.
  • Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients record and providing onward care and information.
  • When and where necessary provide care to patients in routine clinics.

  • Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients record and providing onward care and information.


Homebound Patients

  • Support the organisation, coordination, and delivery of Care to patients that are homebound.
  • Provide a full health check including diagnostics and phlebotomy.
  • Provide Patients with Immunisations both seasonal and long term.
  • Attend the MDTs, recording and following up actions within defined timescales agreed during the meeting.
  • Work under the direction of the PCN Clinical Director and Homebound team, in patients homes carrying our diagnostic procedures on patients.
  • Work with members of the primary care teams, to develop and implement proactive and reactive care and ensure data collection systems are completed accurate and in a timely matter.
  • Raise awareness within the PCN to shared decision making and decision support tools.

There may be, on occasion, a requirement to carry out other tasks; this will depend on factors such as workload and staffing levels:
