The Senior Health Care Support Worker - Physical Health role at Hertfordshire Partnership University NHS Foundation Trust involves providing physical health checks and supporting the overall well-being of older adults with severe mental illness. Working within the Older Adults Community Mental Health Teams, the post holder will assess the physical health needs of service users aged 65 and above in both home and clinical settings. Responsibilities include conducting health assessments, facilitating cardio-metabolic screenings, promoting healthy lifestyles, maintaining SMI registers, and collaborating with multi-disciplinary teams to ensure comprehensive care delivery.

The role requires a strong understanding of the interplay between physical and mental health, as well as clinical guidelines for health monitoring. The successful candidate will engage with service users to create individualized health care plans, promote health education, and document care accurately.
