The North East London Foundation Trust is seeking a motivated Band 4 Rapid Response Assistant Practitioner or Hybrid Worker for their Waltham Forest Rapid Response team in Leytonstone. This role involves providing urgent multidisciplinary care within 0-2 hours to prevent avoidable hospital admissions, working alongside nurses, therapists, social workers, and other health professionals in a community-based setting. The successful candidate will conduct clinical visits in patients' homes, focusing on a holistic approach to care. Flexibility in working hours is essential, as the service operates 24/7, and the use of a personal vehicle is required.

Candidates should have two years of relevant experience in a health-focused role and demonstrate strong communication and interpersonal skills. In-house training will be provided to bridge any skill gaps, and previous experience in a community setting is desirable but not necessary.
