Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.
We offer a 7-Day extended GP access service and a Community Cardiology service and we are looking for a new member of the team to be trained across both departments so that they can assist at busy times where needed. The Community Cardiology Service provides ECGs as part of the diagnostic tests and we are also looking for an individual who would also like to enhance their skills by learning the Health Care Assistant part of the role.
The post holder will be flexible and will be required to travel to different local site locations, sometimes providing both clinical and admin support to different services in the course of one day. Strong communication skills and a compassionate approach for patient facing services are essential for this role.

This role consists of a mixture of patient facing reception duties and administrative secretarial skills to organise and deliver clinics within the Community Cardiology Service and the 7-Day GP Service.
The role involves working across both services based at Family Surgery and the main office at White Moss Way on Southport Business Park.
Reception duties are provided to welcome and process the patient within the Community Cardiology Service and to provide back office administrative and secretarial support to both services on the day.
As well as the reception duties the role also includes training at a Health Care Assistant level within the scope of carrying out ECGs within the Cardiology Service.
The post holder will be flexible and will be required to travel to the two different site locations, sometimes providing both clinical and admin support to different services in the course of one day. About usFull training will be provided and the duties will cross over the following three areas;
Cardiology Service
Provide reception for all clinics, acting as the first point of contact to patients, carers and relatives visiting the community cardiology service, receiving them in a friendly and professional manner.
Review all inbound referrals and exercise judgment to accept or reject if necessary due to generic administrative errors when referral is created.
Process and signpost all qualifying referrals to the appropriate clinical triage or diagnostic team, ensuring that the consultant or technician has the required supporting information.
Contact the patient to arrange an appointment with the appropriate outpatient and/or diagnostic service, coordinating appointments effectively between the different services,
Prepare diagnostic machines for extended ECGs and ambulatory BP monitors prior to each clinic.
Following the return of equipment to the Community Cardiology Service, download extended ECG and ambulatory BP readings for interpretation.
Return diagnostic reports and clinical management plans to the referring clinician/GP in a timely manner.
Deal with telephone enquiries from GPs, patients, and relatives in a courteous and professional manner, and to ensure appropriate messages are passed onto the relevant professionals in a timely manner.
Send patient appointment reminders, feedback questionnaire.
Scan on all Echo results and send to requesting clinician to add recall if indicated
7-Day Service
To receive incoming and initiate outgoing telephone calls in order to facilitate timely and appropriate communications with patients, practices, hospitals and other outside agencies, taking messages, dealing with appropriate queries and raising any queries with the service manager, laboratories and/or responsible director.
Send SMS reminders to all patients with appointments that day/next day.
Generate new SMS templates as and when required.
Action and complete all outstanding tasks from previous evening/session.
Forward tasks to General Practice, followed by a phone call if task has not been actioned.
Manage referrals to other services
Deal with generic emails and action where necessary
Maintain stock levels within the service and order/replenish as required. To ensure all invoices are emailed to S&F Health Finance.
Ensure Family Surgery is up to date with information, clinical rooms are adequately stocked, maintain stock levels in the cupboard and that the stock cupboard is tidy.
Daily handover to evening reception staff and feedback any issues or requirements.
Health Care Assistant Duties
ECGs perform and complete the 7-Day Service administration process for sending results
Decontamination and preparation of ECG / ABPM monitors for the Community Cardiology Service
Programming monitors, uploading patient details, setting the monitors to the appropriate duration, and charging equipment
12 Lead ECGs as requested by consultant cardiologist
Maintaining good standard of equipment and alerting the service manager of any faults
Generating equipment Loan agreements
Generating patient Diaries
Recording equipment details and uploading ABPM results in EMIS records
Uploading ECG results to be processed for analysing
Chaperone duties when required
