Mersey Care NHS Foundation Trust is seeking Band 6 Mental Health Practitioners to join the Crisis Resolution Home Treatment (CRHT) team in St Helens. This role involves providing intensive home support to individuals with complex mental health needs as an alternative to hospital admission, working collaboratively with various services to ensure effective care delivery. Practitioners will be responsible for assessing care needs, developing and implementing care plans, and participating in a 24/7 shift pattern that includes nights and weekends.

The Mental Health Practitioner will engage in face-to-face specialist assessments, manage a caseload, and develop safety plans with service users and their carers. Responsibilities include ensuring effective communication with inpatient units, supporting police colleagues through training and information sharing, and adhering to professional standards in record-keeping and medication management. Practitioners will also contribute to creating integrated care pathways and may act as non-medical prescribers following appropriate training, all while maintaining a focus on user-centered care and collaboration with a multidisciplinary team.
