The Complex Discharge Facilitator position at Herefordshire and Worcestershire Health and Care NHS Trust is aimed at an experienced registered mental health professional—either a Mental Health Nurse, Social Worker, or Occupational Therapist—who is passionate about supporting adults with complex mental health needs. The role involves managing a defined caseload and facilitating smooth transitions for patients returning from out-of-county rehabilitation services to community-based care. Key responsibilities include coordinating multi-disciplinary team meetings, managing discharge planning, ensuring effective care pathways, conducting risk assessments, and advocating for patients during commissioning panels.

The facilitator will work closely with various stakeholders, including commissioning services and Adult Social Care, to ensure timely and safe discharges. The position requires a positive attitude towards innovation and change, strong interpersonal and communication skills, and a commitment to continuous improvement in patient care. The role allows for flexible working hours and entails travel throughout England and Wales.
