The Lead Community Diabetes Nurse position at York and Scarborough Teaching Hospitals NHS Foundation Trust is a pivotal role aimed at enhancing diabetes care within the community. The successful candidate will lead the Community diabetes service, supporting GP staff in improving routine diabetes management and ensuring the completion of annual reviews. This role includes managing the Community Diabetes specialist nursing team, providing expert knowledge and support to healthcare professionals and patients, and visiting practices that are currently underperforming in diabetes care.

In addition to direct patient care and education, the post holder will be responsible for service development in collaboration with the Diabetes and Endocrinology teams, contributing to high-quality audit and research efforts, and leading educational initiatives for primary care staff. The role requires a non-medical prescriber and emphasises effective communication, innovation in care delivery, and a commitment to ongoing professional development.
