Urgent Community Response (UCR)
team! We are seeking a
Band 6 Practitioner
to provide rapid assessments and interventions, helping to keep people safe and supported in their own homes, avoiding unnecessary hospital admissions.
As part of the Urgent Community Response service, you'll work with a multidisciplinary team to deliver high-quality care to patients in crisis, responding within a 2-hour timeframe. This role is fast-paced, rewarding, and essential to delivering integrated, patient-centred care in the community. Our team also supports clinical delivery of the
Virtual Wards (Hospital@Home
) service, providing clinical management to patients with acute illnesses within the place they call home, rather than in a hospital bed.
The post holder will be an experienced Registered Nurse in Adult Nursing or a HCPC registered Paramedic who will work across the UCR services. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.
As a Band 6 urgent community response practitioner you will:
The UCR team works 7 days a week covering shifts between 8:00 until 22:00. You will be expected to work weekends and bank holidays.
