The North Staffordshire Combined Healthcare Trust is seeking a full-time Community Mental Health Nurse for the Greenfield Centre in Stoke on Trent. The role involves providing and supervising mental health care, maintaining care standards, and collaborating effectively within a multi-disciplinary team. The nurse will participate in triaging referrals, conducting holistic assessments, and ensuring that care plans are developed and documented according to assessed needs, while promoting positive relationships with service users and their families.

Additionally, the post-holder will act as a Change Agent, fostering the development of practice and strengthening relationships between different care providers. The role emphasises the importance of teamwork in delivering patient-focused care and ensuring that interventions are appropriate and timely, reflecting a commitment to high-quality mental health services.
