A key part of this role is around supporting the care homes with prevention and management of falls.

Bexley Complex Case Team are looking for an Specialist Physiotherapist to join our Multi Disciplinary Team. We are looking for applicants with a particular interest in Falls and Frailty. We are a Muti Disciplinary Team supporting the care homes in Bexley

The service operates Monday to Friday 9-5

  • Clinical assessments whether as an individual assessment or part of an MDT assessment.
  • Support patients with short term rehabilitation goals.
  • Supporting the Complex Case Team with falls training and clinical assessments of patients following a fall in the care homes.
  • Support care homes with the prevention and management of falls.
  • Clinical support to Complex Case Team staff.
  • Specialist clinical assessment in Care Homes and Community to prevent unnecessary presentation and admission to hospital.
  • Personal CPPD.

About us

The Physiotherapist will work as a part of the multi-disciplinary Complex Case Team to meet the needs of patients referred to the team, ensuring the highest possible standards of practice. The Complex Case Team provide interventions into the Care Homes in Bexley. Our aim is to support residents in the care homes to avoid unnecessary hospital admissions. The team facilitate MDTs with the care homes which include community services, mental health, and GPs to provide interventions required to support the patients in the care homes.

A key part of this role is around supporting the care homes with prevention and management of falls. The post holder will be lead on carrying out assessments following a fall and supporting the care homes to help manage the patients in their current setting. You will also support the Occupational therapist with falls training to ensure care home staff are up to date with relevant requirements.

The Complex Case Team aims to prevent inappropriate admissions to acute hospital wards, unnecessary Emergency Department attendances from the community. The post holder will assess and lead on recommending actions required to help support the patients.
