Service Improvement Lead - Discharge Services inLondon inLondon PUBLISHED 31 OCT 2024

Band 7: £54,320 to £60,981 a year pa inclusive  FIXED TERM 
Alongside the integrated discharge team and the integrated discharge leadership team, identify opportunities for service redesign, promoting a patient centred approach.

An exciting opportunity has arisen as part of maternity leave cover for a motivated, organised and dedicated individual with a passion for service improvement. We are looking for someone with NHS experience, leadership skills and talents for problem solving, data analysis and reporting. This role will manage and support the administration roles within the discharge team and is responsible for ensuring robust provision of administration support to the discharge services and their community partners.

The post holder will also support the discharge Leadership team on a number of exciting service improvement projects including ensuring we maximise digital solutions to process issues using progammes such as Optica and Cerner, our electronic patient record system. Our aim is to improve hospital flow, prevent delays, improve patient experience of hospital discharge and work collaboratively with our community partners.

  • Alongside the integrated discharge team and the integrated discharge leadership team, identify opportunities for service redesign, promoting a patient centred approach
  • Utilise evidence-based service improvement techniques and modernisation tools to develop services in line with Trust objectives
  • Support the integrated discharge leadership team in the development and implementation
  • of business cases and bids to internal and external bodies to improve services
  • Ensure excellent communication with service users, ICBs, GPs and other key organisations in the system is maintained so that their needs and views are taken into account in the planning and monitoring of discharge activities and improvements
  • To prepare and deliver presentations on services to stakeholders
  • Ensure that mechanisms are in place to ensure accurate management of agreed performance metrics
  • Ensure all regular and ad hoc data requests related to the discharge services and inclusion health team service are accurate and delivered on time to relevant internal and external stakeholders
  • Responsible for overseeing the Information Technology needs of the services and ensuring data is captured and analysed in a timely manner and ensuring high standards of patient record management and compliance with the Data Protection Act and patient confidentiality
  • Line manage, coach and develop staff.
