ALCOHOL CASELOAD MENTAL HEALTH MULTI-DISCIPLINARY
Livewell Southwest is seeking a Community Mental Health Support Worker to join their Complex Needs team in Plymouth, providing 22.5 hours of service per week. The role involves supporting clients facing co-existing mental health and substance misuse issues across various settings, including homeless hostels and community locations. The support worker will manage a caseload under the guidance of registered staff, delivering psycho-social interventions, facilitating access to community services, and developing person-centered care plans aimed at promoting recovery and improving overall health and well-being.

In this position, effective communication and collaboration with partner agencies are essential, as is participation in multi-disciplinary team meetings and direct support during drop-in clinics in homeless hostels. The role includes opportunities for ongoing training and professional development, contributing significantly to the community by offering supportive services tailored to the complex needs of individuals living challenging lives. The position will require flexibility, as the post holder may work across different areas within the organisation based on service needs.
