The role of Senior Rehabilitation Assistant at Calderdale and Huddersfield NHS Foundation Trust involves being an integral part of a multi-disciplinary team that provides community-based support to adults in Halifax. The focus is on helping individuals maintain their independence at home, without requiring acute hospital admission. The assistant will implement treatment plans, assist in healthcare activities, monitor patient progress, and support discharge processes while working under the guidance of registered practitioners. This includes participating in initiatives such as the Urgent Community Response Team, delivering care across various settings, and engaging in duties related to equipment management, nutritional assessments, and emergency response.

Key responsibilities include carrying out prescribed rehabilitative interventions, facilitating mobility and exercise sessions, providing personal care, and enhancing patient self-management. The role demands competencies in monitoring clinical data, maintaining cleanliness and infection control, and ensuring the safe operation of healthcare equipment. The successful candidate will also participate in a seven-day service, working on varied shift patterns to deliver timely assessments and interventions tailored to the needs of individuals facing health or social challenges at home.
